1. Acceptance of Terms

By scheduling an appointment, attending a pop-up event, or using any services provided by Renewed Hope Chiropractic ("Practice," "we," "us," or "our"), you ("Patient" or "Client") agree to be bound by these Terms of Service. If you do not agree to these terms, please do not use our services.


2. Services Provided

Renewed Hope Chiropractic offers chiropractic care services, including but not limited to:

General chiropractic adjustments and wellness care

Prenatal chiropractic care

Pediatric chiropractic care

Pop-up community chiropractic events

Dry Needling

Our services are intended to support musculoskeletal health and general wellness. Chiropractic care is not a substitute for emergency medical treatment or primary medical care.


3. Not a Medical Emergency Service

Our services are not designed for medical emergencies. If you or your child are experiencing a medical emergency, call 911 or go to your nearest emergency room immediately.


4. Patient Eligibility & Consent

All patients must complete intake and informed consent forms prior to receiving care.

For patients under 18 years of age (including pediatric and prenatal newborns), a parent or legal guardian must sign all consent forms.

Pregnant patients acknowledge that they have disclosed their pregnancy and any relevant medical history prior to treatment.

You represent that all information provided on intake forms is accurate and complete to the best of your knowledge.


5. Appointments & Cancellations

Scheduling: Appointments may be scheduled online, by phone, or at pop-up events.

Cancellations: We request at least 24 hours' notice for appointment cancellations or rescheduling.

No-Shows: Repeated no-shows without notice may result in a cancellation fee or dismissal from the practice.

Pop-Up Events: Pop-up event availability is subject to change. We will make reasonable efforts to notify attendees of any changes or cancellations.


6. Fees & Payment

Payment is due at the time services are rendered unless otherwise arranged.

We accept [cash, all major credit/debit cards, HSA/FSA].

We are out-of-network with all insurance providers.

Any outstanding balances are the responsibility of the patient or their legal guardian.


7. Health Risks & Limitation of Liability

Chiropractic care, like all healthcare services, carries inherent risks. By receiving services, you acknowledge that:

You have been informed of the potential risks and benefits of chiropractic treatment.

Renewed Hope Chiropractic and its practitioners are not liable for outcomes resulting from incomplete or inaccurate health history disclosure.

To the fullest extent permitted by Tennessee law, our total liability to you for any claim arising out of or related to these Terms shall not exceed the amount paid by you for the specific service giving rise to the claim.


8. Privacy & Protected Health Information

We are committed to protecting your privacy. Your health information is collected, used, and disclosed in accordance with our HIPAA Notice of Privacy Practices, which is provided to you separately. By using our services, you consent to our privacy practices as described therein.


9. Photography & Social Media

From time to time, we may take photos or videos at pop-up events or in our office for educational and marketing purposes. By participating in our services, you grant us permission to use non-identifiable images. If you do not wish to be photographed, please notify a staff member at any time.

If you consent to being featured in identifiable photos or testimonials, a separate written authorization will be obtained.


10. Third-Party Links

Our website may contain links to third-party websites (including our Squarespace booking platform or partner resources). We are not responsible for the content, privacy policies, or practices of any third-party websites.


11. Changes to These Terms

We reserve the right to update these Terms of Service at any time. Changes will be posted on our website with an updated "Last Updated" date. Continued use of our services after any changes constitutes your acceptance of the revised terms.


12. Governing Law

These Terms of Service shall be governed by and construed in accordance with the laws of the State of Tennessee, without regard to its conflict of law provisions. Any disputes shall be resolved in the courts of Knox County, Tennessee.
